Submit your immunization records through the

1. Log in using your CAS credentials
2. Choose "My Forms" and select "Immunizations"
3. Enter the dates of your MMR vaccines
4. Upload a copy of your vaccine record
5. Select "Submit." Please allow up to 24 hours for compliance to be processed.

Need a vaccine or titer test?

• MMR vaccines and titer tests (a blood test showing immunity) are available by appointment at the Student Health Center.
• If you already have MMR titer results, upload your lab report under the Upload Documents tab in the Student Health Portal.

Need a copy of your immunization records from the Â鶹Ãâ·Ñ¸ßÇåÎÞשÂëÇøHealth Center?

If possible, try to locate your original immunization record before contacting us. If you can’t find it, you can request a copy from the Student Health Center by submitting the form below.

Please allow 3–7 business days for processing. We’ll scan and email the record to you, and you’ll be responsible for forwarding it to the appropriate school, provider, or agency.

You have the legal right to opt out of MMR vaccination by filing a non-medical exemption. However, in the event of a measles, mumps, or rubella outbreak, you may be excluded from campus for up to 3 months.

To file a non-medical exemption in the Student Health Portal:

• Option 1: Upload a signed Non-Medical Exemption Form from a licensed immunizing provider, OR
• Option 2: Parents/guardians, emancipated students, or students 18 years or older have the option of obtaining a nonmedical exemption without a provider's signature by completing the  and obtaining a .
