Immunization requirements at FLC
You must submit your immunization record by the Census Date of your first term (fall, spring, or summer). If you do not comply:
Colorado State Law requires all college students born on or after January 1, 1957 to provide proof of two MMR (measles, mumps, and rubella) vaccinations. You must submit your immunization record to the Student Health Center as a mandatory requirement.
Colorado law also requires Fort Lewis College to provide meningococcal (meningitis) disease information to all incoming first-year students living on campus.

If possible, try to locate your original immunization record before contacting us. If you can’t find it, you can request a copy from the Student Health Center by submitting the form below.
Please allow 3–7 business days for processing. We’ll scan and email the record to you, and you’ll be responsible for forwarding it to the appropriate school, provider, or agency.
Ask your physician or previous school to complete the form below and submit your immunization documentation directly to us.
You have the legal right to opt out of MMR vaccination by filing a non-medical exemption. However, in the event of a measles, mumps, or rubella outbreak, you may be excluded from campus for up to 3 months.
