Discover employee benefits at Fort Lewis College

Accessing Your Health Coverage Tax Form (Form 1095-C)

Fort Lewis College provides access to your Form 1095C (Employer鈥慞rovided Health Insurance Offer and Coverage) electronically through Workday. This form is available to be viewed or downloaded through Workday or upon request.

Access Your Form in Workday

To view and download your Form 1095鈥慍 through Workday:

  1. Log in to Workday
  2. Navigate to your Profile Page (person icon in the top right鈥憆ight corner and select View Profile)
  3. Under your name, click Actions, hover over Benefits, scroll to the bottom, and select View My ACA Forms
  4. Select the applicable tax year and download, save, or print the form for your records

Request a Paper Copy (No Cost)

You have the right to request a paper copy of your Form 1095-C at no cost. Upon request, Fort Lewis College will furnish a1095-C at no cost. Upon request, Fort Lewis College will furnish the form within 30 days in accordance with applicable IRS requirements.
