I, the student, understand that the Talent Search program has a strong commitment to education and is here to help me do well in school. To be a part of the program, I agree to do the following:
- Attend school regularly.
- Work earnestly on all my studies and turn in my homework assignments.
- Maintain at least a 2.0 or C average in my core classes (math, science, English, social studies, foreign languages) to be eligible for the program and field trips.
- Have respect for myself as an individual and for other people and their differences.
- Attend Talent Search activities and workshops at least twice a year.
- Graduate from high school.
- Work toward admission into college or vocational school after I graduate from high school.
- Attend tutoring sessions as needed.
- Enroll in a college or vocational school of my choice the semester after I graduate.

I understand that this release of records and personal information will remain in effect throughout my student鈥檚 middle school, high school and future entry into post-secondary education. I also give Talent Search permission to use my child in promotional materials such as newsletters, brochures and/or educational videos. Additionally, should my student require medical attention while participating in Talent Search activities and I cannot be contacted, I give my consent to medical examination and treatment deemed necessary by the attending medical professional. I hereby verify that all of the information reported on this form is true to the best of my knowledge. I understand that all the information I have released will be kept confidential by the Fort Lewis College Talent Search program.
