Use this form if you are a former student who has not attended Fort Lewis College for at least six consecutive terms (e.g., Spring, Summer, Fall, Spring, Summer, Fall) and whose cumulative Fort Lewis College GPA is below 2.00 for undergraduate students or below 3.00 for graduate students. Please note that students are eligible for Academic Renewal only once.

I have not attended Fort Lewis College for at least two academic years, which is six consecutive terms (Fall, Spring, Summer, Fall, Spring Summer).
I understand that, if my petition for Academic Renewal is approved, grades I received prior to renewal will not factor into my cumulative GPA, but they will still appear on my transcript.
I understand that, if my petition for Academic Renewal is approved, previously completed coursework for which I received a grade of C- or higher will count toward graduation if I am an undergraduate student, and previously completed coursework for which I received a grade of B- or higher will count toward graduation if I am a graduate student.
I understand that I can by granted Academic Renewal only once.
I understand that Academic Renewal applies only to my 麻豆免费高清无砖码区academic record, and that it does not apply to the Financial Aid Satisfactory Academic Progress Policy. (If you are planning to apply for financial aid, it is critical that you meet with a financial aid counselor regarding how Academic Renewal impacts financial aid.)
